Trail to Scouts: Adult Information Form

Completed BSA Medical Forms Parts A and B and a copy of medical insurance card(s) are required at check in for all participants.  Medical Forms Parts A and B can be found here.

Additionally, the following documentation is required for adult participation, which can be emailed in advance to coordinator@trailtoscouts.org or turned in during registration:

1. Copy of the adult’s current Youth Protection Training (YPT) certificate. Training may be completed at Youth Protection | Boy Scouts of America (scouting.org)
2. Adults who have consecutively lived in PA for the last 10 years or more:
   • Pennsylvania State Police Criminal Record Check
   • Pennsylvania Child Abuse History Clearance
   • LHC PA Resident Volunteer Verification Form
3. Adults who have not consecutively lived in PA for the last 10 years or more:
   • Pennsylvania State Police Criminal Record Check
   • Pennsylvania Child Abuse History Clearance
   • FBI Criminal Background Check

Absence of completed medical forms (including insurance card(s)) and the above-required documentation will result in you and your scout being unable to participate in the program, without refund.
